University Hospitals publishes a systemwide visitor policy that applies here: visiting hours at most facilities are 7:00 a.m. to 9:00 p.m., visitors arriving after hours enter through the Emergency Department and check in with security or registration, visitors under 18 must be accompanied by an adult who is not the patient, and all visitors sign the visitor log. UH also notes that particular areas may carry their own restrictions. Published hours are when visitors may generally come; whether this patient is on the unit and able to take part at that hour is a separate question only the unit can answer, so ask the unit as well and ask again close to the appointment.
The systemwide hours are the starting point, and the unit desk is what confirms the rest. Ask whether the patient is expected to be on the unit at that hour, and whether the area has its own restriction.
Identity can come from the signer's own acceptable identification or from a credible identifying witness who appears and swears to it. What does not work is arriving with neither arranged. Not everyone can serve: ORC 147.50(D) excludes a witness with a direct financial or other interest in the transaction, or who is named as a party to it. Merely being mentioned somewhere in a document is not automatically disqualifying, so check with us before arranging one. It is a different role from a witness the document itself calls for.
Do not count on hospital staff being available or willing to witness a signature.
Medication and procedures affect alertness. Families usually know the window when a patient is most able to take part.
Only the signature and the notarial certificate should be outstanding when the notary arrives.
The notary must be satisfied that the signer understands the document and is signing of their own free will, and will decline if that cannot be established at the bedside. The hospital controls access and the patient’s care comes first, so an appointment can be delayed or prevented for reasons outside anyone’s control.
